Ashley recommended pumpkin and ground turkey to a client at the ER hospital whose pet required a bland diet for diarrhea. These are the foods we recommend at my practice, but in response to her suggestion, Ashley's ER clinic supervisor gave her the following note. It describes the clinic's official recommendation for patients with diarrhea, plus a hand- written explanation as to why Ashley should no longer recommend the bland diet she learned from me: The subject of fiber is certainly confusing, even for professionals like the veterinarian who drafted these suggestions, but a basic knowledge of the different types of fiber is important in understanding what effect each type will have on an animal's body. Most medical doctors and veterinarians agree that a healthy diet consists of fresh foods that are rich in antioxidants, phytonutrients, and fiber. But the amount and type of fiber a particular species needs has been more difficult for the medical community to agree on. Fiber Defined. Fiber (the tiny threadlike structures in fruits, vegetables, and grains) has historically been defined as the remnants of plant cells that are resistant to digestion, which includes lignans, cellulose, and the indigestible carbohydrates found in plants. However, by definition this would omit indigestible carbs found in animal sources, such as chitin, as well as fructooligosaccharides (FOS) and other digestible carbs that are resistant to an animal's digestive enzymes. For this reason, in 2. Institute of Medicine developed definitions of fiber that distinguished between fiber that naturally occurs in foods (. Some examples of specific types of dietary fiber found in plants are cellulose, hemicellulose, lignins, beta- glucans, and resistant starches (found in bananas and legumes). Examples of functional fiber are inulin, oligofructose, plant gums, and pectins. Fiber can also be classified as viscous and nonviscous, based on its consistency when mixed with water. If the fiber gels in water, as pectins, beta- glucans, psyllium, and some gums do, it's considered viscous. Viscous fiber slows gastric emptying time, can delay the absorption of some nutrients (including sugars) in the small intestine, and lowers cholesterol. Soluble and Insoluble Fiber. Fiber is further classified as soluble and insoluble. Soluble fiber, such as beta- glucans, gums, most pectins, and psyllium, disperses easily when stirred into water. Cellulose and lignins do not disperse in water, so they're classified as insoluble fiber. Research shows that a fiber's solubility does not predict its effect on the body, as previously thought. Soluble fibers bind with fatty acids and slow digestion, which can have a stabilizing effect on an animal's blood sugar levels. Soluble fiber has also been proven to help lower cholesterol in humans. Both insoluble and soluble fibers can be fermentable, and most whole plant fibers contain both soluble and insoluble fiber. Because of this, insoluble fiber prevents constipation and keeps pets regular. In my experience, almost all pet owners and many vets (including the one who chastised Ashley for recommending pumpkin for diarrhea) lump all fiber into this category, assuming all fiber creates the same laxative effect in the gut. However, some fiber, such as the pectins found in bananas, is actually binding and potentially constipating to mammals. Banana pectins draw water out of the feces, putting it back into the body. When water leaves the colon, harder stools are the result. But in the case of diarrhea, adding a small amount of mashed banana to your dog's bland diet (if he'll eat it) can often actually help reduce the incidence of loose stools. Fermentable and Nonfermentable Fiber. In addition to solubility and viscosity, fiber can also be classified as fermentable, partially fermentable, and nonfermentable. Fermentable fiber sources, such as pectins, beta- glucans, guar gum, inulin, and oligofructose, provide a food source for the billions of bacteria naturally found in your pet's GI tract. Some fiber sources, such as cellulose and lignin, are nonfermentable. In general, fruit and vegetable fibers are fermentable and grain fibers are nonfermentable. Current fiber research is focused on the actions and influence of certain types of fermentable fiber in feeding beneficial bacteria in the GI tract. The GI tract is the largest immune organ in the body. GI lymph tissue, called Peyer's patches, as well as Gut Associated Lymphoid Tissue (GALT) is impacted by the balance and health of the microbial microenvironment. Scientists are evaluating how foods can help heal or harm this critically important bacterial balance within your pet's gut, and fermentable fiber can play a huge role in modulating your pet's GI defenses for the better. Why I Recommend Pumpkin Over Rice as the Foundation of a Bland Diet Canned pumpkin (1. Pumpkin is especially rich in soluble fiber (the type that dissolves in water to form a viscous gel, which also coats and soothes irritated bowels). Soluble fiber delays gastric emptying, slowing down GI transit times (and the number of episodes of diarrhea). When animals have diarrhea, they can lose important electrolytes, including potassium, which puts them at risk of dehydration. Hypokalemia, or low potassium levels, can result in cramping, fatigue, weakness, and heart rate irregularities. Pumpkin happens to be an excellent source of potassium, with 5. Pumpkin is also safer for diabetic patients. Unlike rice, which is a grain, and will ultimately break down into sugar, pumpkin extracts may actually restore beta cell function – beta cells are the cells that produce insulin in the pancreas. Rice is a bland source of fiber, but in my opinion, it isn't the most species- appropriate choice for a recovery diet for carnivores. First, it's an unnecessary food. Dogs and cats don't have a nutritional requirement for grain, so feeding pets a pro- inflammatory food when they're already having GI upset seems counter- intuitive to me. Additionally, the FDA has issued a potential warning about arsenic loads in white rice. I have many new client visits that occur when a pet's diarrhea was supposed to clear up with a local vet's recommendations, but didn't. I've had many people tell me that although the stools were slightly improved on a homemade diet of cooked rice and hamburger, the rice would often be passed out whole in the stool, giving rise to the question of how much digestion and absorption of this carbohydrate was actually taking place. Ground Turkey vs. Ground Beef: The Battle of the Burgers. My reason for recommending turkey is simple: hamburger has more fat, which can worsen GI upset, and boiling ground beef doesn't substantially decrease the fat content. Boiling the meat for a bland diet is important because it's the cooking technique that removes the most fat. Meat must reach 4. Boiling water only reaches a temperature of 2. The remaining fat can exacerbate pancreatitis and GI symptoms. Baking the meat at 4. Rinsing boiled or baked meat removes surface fat, but it can't remove the fat that remains in the flesh. For this reason, I recommend fat- free meat for bland diets. You can easily find fat- free ground turkey or turkey breast in most grocery stores, along with 1. NOT pumpkin pie filling). Organic canned pumpkin is also becoming more readily available. Clean Foods Are Best, Plus. So if you prefer, you can certainly buy fresh, organic pumpkin, steam or boil it, and can it or freeze it for later. If you also have access to fresh, organic turkey meat, even better. On occasion clients will say, . If your pet's diarrhea doesn't resolve in 4. If a bland diet resolves the diarrhea, transition your pet back to his regular food 2. Carbohydrates, and the role they play in a healthy diet, are one of the most hotly contested nutritional debates in the world, both in conventional and ancestral health circles. One one side, you’ve got folks who say that carbohydrates are nonessential and increase your risk for diseases such as diabetes, cancer, and neurological disorders. On the other side, some say that carbohydrates are crucial for good health and should make up the majority of your calories. It’s no wonder I have so many clients who come to me completely confused about carbs and whether or not they should eat more or less of them. While some people do incredibly well following a low carb Paleo diet, there are many people who crash and burn on this type of dietary plan. So, how can you tell if you’re the type of person who shouldn’t be eating a low carb diet, and how do you figure out how many carbs you should be eating? I plan to teach you just that in this article.
Is your low- carb diet doing you more harm than good? Find out how many carbs YOU should be eating. Pregnancy. I recently discussed the role of carbohydrates during pregnancy in an episode of The Ancestral RDs Podcast. The most important reason why women need adequate carbohydrates during pregnancy is to ensure adequate fetal brain development and growth. Another reason is because a high protein diet can be dangerous during pregnancy, and when you cut out carbohydrate as a major macronutrient, you usually can’t help but increase protein as a percentage of calories. Protein intake greater than 2. The Institute of Medicine recommends a minimum of 1. ![]() Paul Jaminet advises pregnant mothers to restrict protein to about 1. Chris recommends a moderate carb approach for most pregnant women (except those with any type of diabetes) in his book, Your Personal Paleo Code (published in paperback as The Paleo Cure in December 2. Like these other experts, I generally recommend 3. Unless you have diabetes or a serious neurological condition that requires carbohydrate restriction, it’s not worth the risk playing around with a low carb diet when you’re pregnant, and these diets aren’t conducive to fertility for many women either. Each athlete is completely unique in their ability to perform well on a low carb diet, and there’s nothing wrong with testing out the diet to see how it affects your athletic ability. But if you’ve been trying a low carb diet for months now and your workouts are suffering, your weight isn’t budging (or maybe you’ve even gained weight!), and your recovery time is increasing, you’re probably not the type of person who can handle a low carb diet combined with regular intense physical activity. I’ve had many clients come to me on a low carb diet who, after switching to a more moderate carb approach, found that their energy and endurance significantly increased, and they were able to make quicker strength gains than before. Many also were able to shed some of the stubborn body fat that they’d been retaining despite eating a low carb diet and training hard, which was a result they didn’t expect! For my athletic clients, I usually recommend a minimum of 2. I may actually recommend more like 4. Again, each athlete is an individual and what works for one person, or even a thousand people, may not work for you. So don’t be afraid to experiment and pay attention to how your diet makes you look, feel, and perform! And don’t hesitate to get help if you need it! Hypothyroidism and HPA Axis Dysregulation (Adrenal Fatigue)Hypothyroidism is one of the most commonly cited medical reasons for needing to eat a moderate carb diet. The main reason why carbs affect thyroid function so directly is because insulin is needed for the conversion of the inactive T4 hormone into the active T3 hormone, and insulin is generally quite low on very low carbohydrate diets. So if you’ve suddenly started developing hypothyroid symptoms on your low carb diet, it’s a pretty good sign that you’d be better off upping the carbs (and getting your thyroid tested if you haven’t already!) HPA axis dysregulation, also known as adrenal fatigue, is another condition where a moderate carb intake is important for general health. Kelsey and I talked about adrenal fatigue on our first Ask the RD podcast, so listen to it if you’re unfamiliar with this condition. The main hormone that gets dysregulated in adrenal fatigue is cortisol, and cortisol has been shown to increase on a low carb diet. This means that a low carb diet is a potential adrenal stressor in susceptible individuals. Combine that with a stressful job, inadequate sleep, and overexercise, and you’ve got yourself a recipe for adrenal burnout. So if you have adrenal fatigue, or if your current lifestyle is already high stress in a few different areas, you may want to increase your carb intake until you can get those additional stressors under control, as you may drive yourself into adrenal fatigue by having a chronically elevated cortisol output. Lara Briden has written a great article on the benefits of whole food carbohydrates in lowering cortisol and raising GABA, a calming hormone that is often low in adrenal fatigue patients. If you’re experiencing symptoms of hypothyroidism or adrenal fatigue, you may be worsening them with a well- intentioned low carb Paleo diet. I recommend working with someone to help you figure out if your diet is indeed making these symptoms worse, and how to change what you eat to better support your thyroid and adrenal health. Gut Health. One of the less discussed downsides of a very low carbohydrate diet over the long run is the potential for alteration of the gut flora. Chris recently covered this issue in a podcast with Jeff Leach, where they discussed evidence that a very low carb diet can lead to gut dysbiosis and a reduction in the diversity of the gut flora. A lot of the information on this topic is new and not fully understood, but it’s reasonable to believe that when you avoid carbs, you’re also avoiding important prebiotics (i. Without them, your beneficial flora can’t produce as much gut- healing substances like butyrate and other short chain fatty acids, and your microbiome composition may even shift in an undesirable direction. And as Chris would say, you’re only as healthy as your gut is: an unhealthy gut contributes to everything from obesity and diabetes, to digestive illness, to autoimmune disease, to skin disorders. Those who are doing very low carbohydrate diets, and who simply can’t increase their starch intake for whatever reason, should use prebiotic supplements such as resistant starch- rich unmodified potato starch or FOS powder. However, these products must be incorporated slowly into your supplement regimen, as you can experience severe gas and bloating if too many prebiotics are taken all at once, or if there is existing gut dysbiosis or bacterial overgrowth. In this case, it would be wise to work with someone who can help you get the prebiotics you need while on a very low carbohydrate or ketogenic diet to protect the health of your gut microbiota. How Many Carbs Do I Need? To determine how many carbs my clients need to eat in a day to reach their health goals, first I decide what percentage of calories from carbs they’d possibly do best with. As an example from above, a woman struggling with fertility issues may benefit from a carb intake around 3. I then determine her caloric needs using a calculator like this one. If we determine that her daily needs are roughly 2. As there are 4 calories in a gram of carbohydrate, this works out to be 1. Usually I give my clients a target range to hit depending on their activity levels, and we use these targets to re- evaluate their food diaries and see if they’re hitting their goals. Most of my clients are surprised to learn how much carb- rich Paleo foods they have to eat to get up to 1. But once they start making a real effort to hit those targets, the health benefits are immediate. These benefits include weight loss, elevated mood, skin improvements, increased energy, normal menstrual function, more satisfying sleep, and more. It’s so exciting to see what a couple of starchy tubers and pieces of fruit can do for a person’s health when they simply need to eat more carbs! The purpose of this article was not to bash low carb diets. I truly believe that there are many people out there who get amazing health results from a low carb plan, and there are dozens of health conditions that benefit from a very low carb or ketogenic diet, especially severe neurological conditions.
